In the event of an emergency, who will make health-related decisions for you?

Everyone 18 and older is encouraged to complete an advance directive. This care planning process includes designating a health care surrogate who will speak on your behalf if the need arises, and the completion of a living will.

In honor of National Healthcare Decisions Day in April, we encourage you to complete an advance directive to help you plan for unforeseen medical circumstances and end-of-life care.
