SUPPORT MEDICATION SAFETY — PLEASE REPORT CONCERNS

We need your help! Our ability to identify medication safety events relies heavily upon voluntary reporting by employees and physicians using the online patient safety reporting system, IDinc.
IDinc can be accessed quickly on the ShandsConnect portal (see below). Each patient safety event entered is automatically sent to the unit manager where the event occurred and to the Quality Management Department. Reported events are tracked and trended to help us identify opportunities for improvement.
Analyses of medication events focus on system problems rather than on the individuals directly involved with an incident. Medication errors that resulted in potential patient harm (near misses) or actual patient harm are reviewed monthly by the Shands at the University of Florida Medication Safety Committee. The committee’s focus is to identify trends with certain medication classes, processes or teams and work with key groups to develop action plans and improvements.
Health care organizations with a strong culture of safety have high rates of voluntary safety reports. As we continue on our journey to improve patient safety and quality care, we expect to see our voluntary reports increase. Please participate and take time to enter a patient safety report into the IDinc system for any situation that could, or did, result in sub-optimal care for our patients.
