    • Explanation of medication side effects
    • Quietness around patient rooms

  • Increase use of home care and telehealth
  • Work with community-based organizations to increase postdischarge support
  • Enhance physician follow-up after discharge
  • Implement best practices communicated by the University Health System Consortium, the Institute for Healthcare Improvement, and the Florida Hospital Association
